Raritan Family Health Care is recognized as a Level 3 Patient-Centered Medical Home by The National Committee for Quality Assurance (NCQA).
We use a team approach to providing primary health care, including your health care provider, others who support you, and — most importantly — you. We use technology such as our patient portal to communicate and coordinate your care and provide the best possible outcomes for you and your family. Elisa has experience in a variety of settings with clinical specialties in oncology, weight management, cardiovascular disease and diabetes management. Through Medical Nutrition Therapy paired with motivational interviewing techniques, Elisa has helped individuals sustain positive behavior changes to create a healthy relationship with food for optimal health.
Sarah L. Masucci, LCSW, LCADC
Sarah L. Masucci, LCSW, LCADC, is a licensed clinical social worker with certifications in Gerontology and Clinical Supervision. She graduated summa cum laude from Rutgers University Graduate School of Social Work. For the past 15 years, Sarah has worked to improve the quality of life for older adults with complex health conditions. Before joining JFS, she was the National Manager of Summit Medical Group’s Social Support program.
Sarah focuses on establishing creative and collaborate relationships with her clients, families and the community service providers that may be incorporated into each client’s plan of care. Her passion is in working with older adults to achieve the greatest degree of autonomy, functionality, satisfaction and safety in their homes. She has experience working as a clinician, supervisor and administrator in government, nonprofit and privately-owned companies.
Sarah specializes in supportive care and provides counseling services to older adults in their homes.

Jacklyn Coburger MSW LCSW
A graduate of Rutgers University with a Masters in Social Work and training as an Addiction Counselor.
Jackie likes to provide a non-judgmental, safe, warm, and accepting environment. Her goal is to help you live a most fulfilling life. Her areas of expertise include anxiety, depression, personality disorders, life transitions, self-esteem, relationship/family issues, substance abuse, and trauma. She is trained in Cognitive-Behavioral Therapy, Dialectical Behavior Therapy, Trauma-Informed Care, ARC GROW, and Motivational Interviewing. She has worked in public and private sector with youth, adolescents, adults, families, couples, partial hospitalization, in-home, and nursing homes.

Giving doctors extra support to help you get better care
Our goal has always been to provide you with the highest quality of care. Through Primary Care First, we will receive additional resources from Medicare and other health insurance companies to help us enhance our work and provide you the best quality, patient-centered care.
More information for patients with original Medicare
As part of this program, Medicare will start sharing some of your personal health information with us, such as when you receive care at hospitals, emergency departments, and specialist offices. This will help provide us with a more complete picture of your health and allow us to better coordinate your care.
If you want to stop Medicare from sharing this information, you should call
1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.
Your Medicare benefits are not changing, and this program is only intended to enhance the healthcare you receive at Raritan Family Healthcare. You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it is always your choice which doctors you use or hospital you visit.
